Provides skilled nursing care to patients and families in homes, clinics and other community settings in accordance with agency policies and procedures. Promotes, maintains and restores the health of individuals and the community through appropriate teaching, counseling, case finding and rehabilitative measures. Reports to the Supervisor of Clinical Services. EOE
Current license to practice as a Registered Nurse in the State of Connecticut
One year home care experience preferred
