The Intake Coordinator is the first point of contact for the referral source and determines customers’ needs based on incoming referral information. Responsible for accepting cases from referral sources and working with Supervisors to assign professional staff for skilled services and case management provided to the agency’s patients and caregivers.
Hours: 8:00am – 4:30pm Monday through Friday
Per Diem with possible transition to full time
Must be a Registered Nurse, or Licensed Practical Nurse licensed in the State of Connecticut.
Must have completed one (1) year of clinical nursing experience in an acute care or extended care facility. Experience in the home care environment or in case management is preferred.
Familiarity with the State of Connecticut Department of Public Health, Home Health Agency Regulations and the Medicare Conditions of Participation for Home Care Providers regarding the acceptance of patients for therapeutic care is preferred.
Experience as an intake or triage nurse is preferred.
Must have excellent computer skills. Previous work with electronic medical record is preferred.
