Registered Nurse
Provides skilled nursing care to patients and families in homes, clinics and other community settings in accordance with agency policies and procedures. Promotes, maintains and restores the health of individuals and the community through appropriate teaching, counseling, case finding and rehabilitative measures. Hospice Nurse
Gives skilled nursing care to terminally ill patients and their families in homes, clinics and other community settings in accordance with agency policies and procedures. Performs the functions of a primary care nurse with additional hospice duties. Reports to the Hospice Director. EOE … more
Intake Coordinator
The Intake Coordinator is the first point of contact for the referral source and determines customers’ needs based on incoming referral information. Responsible for accepting cases from referral sources and working with Supervisors to assign professional staff for skilled services and case management provided to the agency’s patients and caregivers.
Hours: 8:00am – 4:30pm Monday through Friday
